Community Health Worker Intervention & Prevention Services

The following is a detailed list of all the conditions for which AYO provides intervention and prevention services.

Communication & Breaking Generational Trauma

Interventions:

  • Provide workshops and counseling on improving communication within families and communities
  • Help individuals recognize and address generational trauma, offering trauma-informed care and resources
  • Facilitate support groups for families and individuals dealing with the effects of intergenerational trauma

Preventions:

  • Educate communities on the impact of generational trauma and how it influences behavior and relationships
  • Promote positive communication techniques to prevent conflicts from escalating into trauma
  • Introduce programs that focus on mental health, healing, and building emotional resilience

Filing Complaints about Services

Interventions:

  • Assist individuals in understanding their rights when accessing services (healthcare, social services)
  • Help draft and file complaints regarding inadequate or discriminatory services
  • Liaise with service providers to ensure complaints are heard and addressed

Preventions:

  • Educate the community on their rights and responsibilities regarding public services to prevent misuse or misunderstanding

Housing

Interventions:

  • Connect community members with affordable housing options and emergency shelters
  • Advocate for better housing conditions and tenants' rights
  • Assist in navigating housing assistance programs and legal issues related to evictions

Preventions:

  • Work with local governments to prevent housing shortages and promote fair housing policies
  • Promote community awareness of housing rights and resources

Quality Food Access

Interventions:

  • Help families sign up for food assistance programs (e.g., SNAP, food banks)
  • Provide education on nutrition, healthy eating, and sustainable food sources
  • Organize or collaborate on community gardens and food distribution programs

Preventions:

  • Advocate for local food policies that increase access to fresh and nutritious food

Behavioral Health & Modification

Interventions:

Preventions:

Resource Navigation

Interventions:

  • Assist individuals in identifying and accessing community resources such as healthcare, legal aid, and employment services
  • Provide case management services to ensure follow-up and successful use of resources

Preventions:

  • Conduct resource awareness programs to inform community members about available services
  • Organize resource fairs or community hubs where people can access multiple services in one place

Civic Education & Voting (Non-partisan)

Interventions:

  • Educate community members on the importance of civic participation and voting
  • Help with voter registration and explain the voting process in a non-partisan manner
  • Provide information on candidates and issues in an unbiased, informative way

Preventions:

  • Encourage long-term civic engagement by promoting active community involvement in local governance
